Community Health Center, Inc. of Enfield is offering School Based Dental services to Enfield Public School students during their school day. The services consist of a cleaning, exam, fluoride treatment, sealants and dental x-rays if needed. Proper brushing and flossing techniques are also reviewed.
All children who enroll are eligible to receive services regardless of insurance status.
- There is no charge for services for children insured by husky or Medicaid
- Most insurance plans are accepted with coverage & costs dependent on the plan.
- For patients with NO dental insurance, the cost for the services are $30.00 and the cost for sealants is $25.00
All parts of the registration/enrollment form* must be completed, signed and returned to the school before a student may receive the dental services. Click here to download the enrollment form. Please note the CHC Mobile Dental Services form is in both English and Spanish.
