Sports Physicals

Physical Exam Requirements for Sports Participation in Grades 6-12  

To participate in any sport at any level, all students are required to have a physical exam, conducted by their health care provider (physician, physician's assistant, APRN or RN), on record in the Nurse's Office.

This physical exam is good for 13 months.

IMPORTANT NOTE:
Physicals for 6th and 10th grade students only, will only be accepted on the State of Connecticut Blue Health Assessment Form 
available in the Main Office of both EHS and JFK.

The physical for 6th and 10th graders counts for a sports physical as well.  They only need to turn in that one form to the nurses.

This physical must be on record PRIOR to try-outs: there are no exceptions.

If a student's exam expires in the middle of the particular season, he or she may begin the season but must have an updated physical by the expiration date.

Points to Remember:

Due to the large number of students who participate in sports, it is often difficult to get an appointment with your health care provider on a last minute basis.  Parents whose children plan to participate in a sport are urged to make an appointment with their health care provider well in advance of the intended sports season.

The Nurse’s Office or Athletic Office will be glad to help you clarify any matter relating to sports physicals.

Sports Examination Form

The Sports Evaluation Physical Form can be picked up Enfield High School in the main office.   You can also find a copy of the physical form on the following website. (you will only need pages 1 & 3)   http://www.casciac.org/pdfs/physical.pdf

The following dates and times have been set aside in the summer to process updated physical forms in the nurse’s office at Enfield High.  You can also come in and drop them off in a box in the main office at Enfield High anytime.  No need to wait for the nurses to be in the building.  We have copies of the physical forms in the main office as well.


Physical forms must be turned in and processed by the School Nurse. 

Coaches will not accept and cannot approve the validity of this information.

(Middle School physicals can be mailed or dropped off once school begins.)

Monday, August 6, 2018 - 11:00 a.m. to 4:00 p.m.

Friday, August 10, 2018 - 12:00 p.m. to 5:00 p.m.
