Lefebvre Insurance LLC, and ACE American Insurance Company will be handling the 2011-12 Student Accident Insurance program for Enfield Public School students.  Listed above is a link that includes the enrollment form with instructions, a claim form with instructions and instructions for mailing completed 2011-12 Student Accident Medical Insurance forms.
